package reflects that fact. Following is a summary of Valley Academy’s
2011-2012 benefits package.
Retirement
Valley Academy provides retirement and long term disability benefits through the Arizona State Retirement System. This is a valuable benefit to our employees. Only one-third of charter schools in Arizona offer ASRS retirement, and we are happy to provide this benefit to our staff. ASRS guidelines dictate eligibility and participation rates. This benefit is available to employees who regularly work over 20 hours per week. Valley Academy matches 100% of employee contributions.

A supplemental Salary Deferral Plan is offered in conjunction with the Arizona State Retirement System through TIAA-CREF for employees who wish to have an additional way to save for retirement.

Health Insurance
Valley Academy offers comprehensive medical coverage through either a United Healthcare PPO or a HDHP with a Health Savings Account (employees may choose between these two options). Employees pay only $10 per pay period for health, dental, and/or vision insurance – the school pays the rest. Coverage for dependent spouses and children may be purchased for an additional charge. Employees who regularly work over 30 hours per week are available for this benefit.

Dental Insurance
Employees receive comprehensive dental insurance through Total Dental. This coverage is offered for $10 per pay period and is free for employees who opt for health or vision insurance coverage. Total Dental offers a wide variety of dentists. For an additional fee, employees may opt to purchase a premium plan, increasing coverage options.

Vision Insurance
Valley Academy employees receive contacts, glasses, and free annual eye exams through our excellent vision program. Like the dental insurance, this coverage is offered for $10 per pay period and is free for employees who opt for health or dental insurance coverage. Sight Care restricts coverage to Nationwide Vision Centers, which are conveniently located throughout the valley.

Life Insurance
Valley Academy provides $25,000 in life insurance to every employee, regardless of hours of employment. Additional insurance for employees, spouses, and dependents may be purchased upon request.
AFLAC Insurance
Many additional insurance packages are available for purchase through AFLAC Insurance, including Short Term Disability, Hospitalization insurance, Cancer Coverage, and more.
